Daily Personal Activities

We offer hygiene and personal care and domestic activities assistance to participants in their home such as: Showering, Toileting, Changing, etc.This support aims to develop the participant’s daily life skills to help them uphold independence.

Assistance with Travel/Transport Arrangements

We offer transport participants to any activities outside of the home, such as: To and from appointments or a workplace, Specialised transport to an educational facility or community destination.

Community Nursing Care

We offer community nursing care to participants in their home to respond to the disability-related health needs of a participant where that care is not the usual responsibility of the health system.
This is where participants require higher levels of care and support and may involve
assistance with: Personal care, medication administration, supporting participants in
the use of personal equipment including standing or walking frames, wheelchairs,
hoists, sliding sheets or moving boards, hearing aids and other physical aids.

Assistance with Daily Life Tasks in a Group or Shared Living Arrangement (SIL)

We offer participant’s home and personal care in Specialist Disability Accommodation (SDA)and shared living arrangements.
Responsibilities of the provider may involve supervision or support with Personal care; Personal hygiene, Showering and/or toileting, Dressing, Mobility, Appointment
assistance and/or Short term accommodation

Innovative Community Participation

We offer participants the opportunity to build skills and to offer the opportunity to become actively involved in their community. These opportunities may include the participant’s involvement in volunteering, charity organisations, neighbourhood associations or any other innovative activity in the
community.

Development of Daily Living and Life Skills

We assist the participant in developing individual life skills and training around community, social and recreational activities (in order to promote
as much independence as possible). This may involve training in daily personal
activities and important life skills, such as: Public transport skills, Resume skills, Taxes,
Saving skills, Social skills, Physical activity skills, etc.

Household Tasks

We assist participants within their home, which may involve undertaking essential household tasks the participant is not able to complete. Providers maybe responsible for: Cleaning, Gardening, Basic indoor/ outdoor home maintenance, Laundry/ linen, Meal preparation, etc.

Participation in Community, Social and Civic Activities

We support or supervise a participant to take part in community, social, recreational or economic activities. This is provided in a range of environments, such as: In the
community or at a centre. Taking participants on shopping trips or any appointments; Leisure activities, etc.
This service category can also provide support that encourages skill building
regarding the community participation potential.

Group and Centre Based Activities

Services enabling providers to support participants with socialisation and community
participation either in a centre-based setting or directly in their community. These
supports aim to provide participants opportunities to engage in activities and events in a group, while being appropriately supported when doing so. Group activities may
include Dance classes, Cooking classes, etc and centre-based activities.
